Heather Locklear is seeking treatment for depression and anxiety at an undisclosed Arizona facility, People reports. The 46-year-old stunner "requested an in-depth evaluation of her medication and entered a medical facility for proper diagnosis and treatment," her rep tells "Extra."

In March, a mysterious 911 call from Heather's home set off frenzied Internet speculation that the former "Melrose Place" star had tried to take her own life. Her rep immediately shot down those reports, telling "Extra," "She never requested medical assistance and did not place a 911 call. Nor did anyone from her house call 911 or place a call requesting medical assistance."

Officials confirmed that police responded to Heather's L.A. home for a suicidal subject, but the 911 call didn't originate from her home -- and police said paramedics didn't find a problem when they arrived.
